Please follow these step by step instructions.

Any delay in applying for your certificate will hold back your employment with DNA Forensic Nursing Agency Limited.

• You will need to contact the Criminal Records Bureau as soon as possible on 0870 909 0844

• You will be asked to quote a reference number. The number you need to quote is: 22601100000 along with our organisation name D'N'A Forensic Nursing Agency Limited

• You will beasked to make a payment by either a Credit or Debit Card or you may need to send a cheque along with your application when it is thoroughly complete.

A disclosure application form and guidance booklet will be sent to your home address for you to complete. It is important that you read the guidance booklet carefully as only certain sections need to be completed.

When you have completed your application we will have to check the information supplied before you can send it back to the Criminal Records Bureau. You can either:

• bring your application along to your interview along with the documents stated below**

• Or you can send us your application along with your documents**.

Once it has been checked we send it back to you so you are able to forward it on to the Criminal Records Bureau.

**We will need to complete a section of your application. In order for us to do this you will need to provide the following documentation with your application (THESE WILL NEED TO BE THE ORIGINAL DOCUMENTS. PHOTOCOPIES WILL NOT BE ACCEPTABLE):

• 2 proofs of identification
(Passport, Driving Licence, Birth Certificate)

• 2 proofs of address
(Utility Bill, Bank Statement, Television Licence)

Please ensure your application is completed thoroughly and all previous address history for the last 5 years is included along with any time spent abroad, making sure all dates correspond and are consecutive.
